2018 ESL Team Form

With the 2018 ESL season upon us, we ask that each team read, fill out and submit this form no later than October 26th at the front desk indicating:

• The team name
• The team captain
• The names of each player on the team
• How you want to split up the team fee
• Everyone’s initials
